If I use a NMIS-4004 board on the NMIP-9100 board, do I still address the 4004 with the 16 pin, dual position jumpers, or does the NMIP-9100 require special addressing?

Yes, you have to choose an address in 64K I/O space jumpers of the NMIS/L-4004 where is not conflict with any other peripheral on your system (PC/XT).
eg. If you choose to use the system I/O address from 0000 to 03FF make sure the address you jumper on the NMIS/L-4004 does not conflict with any device such as serial ports, parallel ports, mouse, etc... or use the system memory address from 00400 to FFFFF make sure the memory address does not use by any operating system or software on your system.
Don't forget to set J21, jumper A & B to IOR/IOW or MEMR/MEMW for I/O or MEMORY read/write accordingly to your selection.
